Social Determinants of Health
Improving health and well-being across America requires addressing critical social circumstances and related environmental hazards that impact health outcomes, commonly referred to as social determinants of health (SDOH). SDOH affect a wide range of health, functioning, quality-of-life, and societal and economic outcomes.
Assessing factors such as poverty, race, affordable housing, income, access to education and employment, and commuting challenges can help develop holistic strategies to create more equitable, healthy, sustainable communities.
Individuals living in communities unable to meet social needs are at higher risk for health conditions such as heart disease, stroke, depression, cancer, and diabetes.
Health outcomes due to unmet social needs may be stratified by geography, race, ethnicity, age, income, disability status, and a number of other factors. Evidence suggests that interventions addressing social needs can improve health outcomes.
